New patients complete a medical history form. It tells us what else you are being treated for and what you are taking, which is what lets us treat around it safely rather than discovering it halfway through.
Arrive ten minutes early and we will give you the form at the desk. Either way it gets done before you go through.
Bring your insurance card and a list of any medication you take. If you would like records transferred from a previous dentist, bring their name and we will request them.
